Social work is a practice based profession and an academic discipline that promotes social change and development, social cohesion, and the empowerment of liberation of people. Medical social work primarily focuses on supporting patients and their families in hospitals, community clinics and other healthcare settings by coordinating patient’s care with the larger medical team. Social worker play an essential role in many of the non-medical aspects of patient care including helping patients and their families navigate the medical system, assessing and monitoring patients and family members mental and emotional health.
Clinical coordinator oversees the day to day operations of a healthcare program or department. They perform administrative duties such as coordinating care teams for patients, and acting as a liaison between patients, patient families and health care professionals.
